WAIVER AND RELEASE OF LIABILITY FOR BUTLER PRO GYM
I, the customer, hereby agree to the following when purchasing a day pass, a 7-day pass, or memberships from Butler Pro Gym:
1. That I am participating in training, personal training, and/or events that Butler Pro Gym offers which I may or may not receive instruction in.
2. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the Butler Pro Gym or under the guidance of an on-staff personal trainer of Butler Pro Gym.
3. In consideration of being permitted to participate in any type of training, I agree to assume full responsibility for any risks, injuries, or damages, known or unknown, which I might incur as a result of participating in Butler Pro Gym.
4. In consideration of being permitted to participate in any Fitness Program offered by Butler Pro Gym and its staff, I knowingly, voluntarily, and expressly waive any claim I may have against Butler Pro Gym, its owners, Landlords or insurers, or any fitness instructor for injury or damages that I may sustain as a result of participating in Butler Pro Gym.
5. I, my heirs, or legal representatives forever release, waive, discharge, and covenant not to sue Bronson Butler, Butler Pro Gym, landlords or insurers, or any fitness instructor for any injury or death caused by their negligence or other acts.
Butler Pro Gym Rules.
Train how you want to train…. loud, shirt off, blasting music, etc – just put your shit away. Put gym equipment back properly. The gym Owner of Butler Pro Gym can refuse service and cancel membership at any time.
Cancellations & Refunds
Customer can cancel their membership at any time. There will be no refunds. The owner of Butler Pro Gym can refuse service and/or cancel membership at any time.
I have read the above release waiver of liability and fully understand its contents. By me purchasing, I understand and agree to the Waiver, Cancellation & Refunds, and Butler Pro Gym Rules and will abide by them.
